General Financial Information
If you have health insurance, we will need a copy of your identification card. When you enter the hospital, we will ask you to sign a form that assigns benefits from the insurance company to Lake Region Healthcare Corporation. If your insurance requires that you seek pre-certification or authorization for services, please make sure you have met those requirements. If you do not follow your plan’s requirements, you may be financially responsible for all or part of the services rendered in the hospital.
If you have Medicare, we will need to see your Medicare card to verify eligibility and to process your Medicare claim. Deductibles and co-payments are the responsibility of the patient.
If you are covered by Medicaid, we will need a photocopy of your card.
The hospital is responsible for submitting bills to your insurance company, and we will do everything possible to expedite your claim. Please remember that your policy is a contract between you and your insurance company, and you have final responsibility for payment of your hospital bill. We will send a statement to you once your insurance has paid its share. Your balance must be paid within 30 days. For your convenience, payment of your balance can be made by cash, personal check, VISA, MasterCard, American Express or Discover. Automatic debit is also available. If you are covered by more than one insurance policy, each carrier will determine how to divide payment(s). If we receive an overpayment on your account, the hospital will make a refund to the appropriate party.
Lake Region Healthcare offers assistance with the MNsure Health Insurance Marketplace enrollment Monday through Friday. Two certified MNsure assisters are available by appointment by calling 218.739.6711 or 218.736.8198. If you do not have insurance, please make payment arrangements with the business office. These arrangements can be made either before your discharge or after. If you would like someone to visit with you before your discharge to discuss arrangements, please call 218.736.8000. Lake Region Healthcare Corporation will not become involved in financial liability disputes. Financial responsibility for services always rests with the patient. We will release records only when there is proper authorization to do so.

Bills are due upon receipt. We offer several payment options for your convenience.
- Pay within 30 days of statement date and receive a 10% discount on owing balance
- Pay within 6 months of statement date

We Accept Payments:
- Online through our Manage My Account link.
- By mail using the envelope and stub on your bill statement.
- Drop box behind the Federal Courthouse building in downtown Fergus Falls.
- In person at the Business Services Center (south of the hospital building at 126 E Alcott in Fergus Falls).
- In person at the Clinic Registration desk at 111 W. Vernon Ave in Fergus Falls.
To consolidate your billing statements for multiple family members, fill out this form to apply for a single monthly statement that includes charges for all family members you designate.
If you have questions about financial arrangement options or would like additional help, contact Patient Financial Services. We are located in the Business Services Center at 126 E Alcott, just south of the hospital building and are open from 8 to 4:30 Monday through Friday. We also have offices in both the main clinic and the hospital for the convenience of patients already in those facilities.
If you are uninsured and your annual household income is less than $125,000, you may qualify for a discount from the billed amount under our self-pay discount program for Minnesota residents. Please contact Patient Financial Services at 218.736.8000 to begin the application and qualification process for the discount or download the terms, conditions and application here.
If you have certain tests or treatments in the hospital, you may receive bills from physicians you do not see in person, including radiologists, pathologists, anesthesiologists and other specialists. These bills are for professional services rendered by these doctors in diagnosing and interpreting test results while you were a patient. If you have questions about these bills, please call the number printed on the statement you receive from them.
Lake Region Healthcare Corporation's Community Care program was established to assist patients who do not qualify for medical assistance and whose annual income is at or below 175 percent of the federal poverty level.
The application is brief and asks for information about family size, employment, income, banking and assets. If patients meet the guidelines, they will qualify for a write-off of 20 to 100 percent of their balance. Patients must apply for Medicaid and be found ineligible before they can qualify for the Community Care program.
