Urine drug screening business account

Follow the steps below to set up a business account with Lake Region Healthcare's lab for urine drug screening (this only needs to be done once and can be completed any time prior to sending your first employee to LRH for services).

  1. Set up a business account.
    • Lake Region Healthcare is a collection site only.
    • Lake Region Healthcare needs a billing protocol mailed, faxed (218.739.6649) or emailed prior to services.

  2. Contact a testing laboratory. Listed are some choices.

Ask the representative how to set up a drug screening account. The employer and Medical Review Officer (MRO) information needs to be pre-printed on the chain of custody forms. Forms, kits and shipping supplies can be stored at LRH or with the employer.

Breath alcohol testing business account

Follow the steps below to set up a business account with Lake Region Healthcare's lab for breath alcohol testing (this only needs to be done once and can be completed any time prior to sending your first employee to LRH for services).

  2. Alcohol testing forms (ATF).
    • LRH has DOT and NON-DOT forms.
    • LRH requires that a Designated Employer Representative (DER) phone number be on file.

Authorization forms

We must have a completed and signed employer authorization form for any patient coming to Lake Region Healthcare's lab for drug urine screening or breath alcohol testing. We must also have a completed and signed employer authorization form for any patient coming to Lake Region Healthcare for an employment-related physical or for treatment of a first-time or new injury. This authorization form—completed and signed—is crucial: We cannot treat patients without it.

Any employee being tested for reasonable suspicion must be accompanied by a supervisor.

Other important documents

Photo identification: All patients must show us their photo identification before treatment. Acceptable photo identifications include a driver's license, work badge or passport.

Treating minors

A parent or legal guardian should accompany the minor. If you are not the legal guardian, you will need a notarized statement signed by a parent or the legal guardian authorizing you to give permission for services and billing, if appropriate.
